Registration is now closed. Thank you to all who registered. Subsequent registrations will be handled onsite in Seattle, Washington on a space available basis.

Academic: 

 

Registrants who are employed by a university, college, research institution or government agency.

Student:

 

Registrants who are enrolled in an undergraduate or graduate program at an academic institution.

Corporate:

 

Registrants who are employed at a company.

The symposium registration fee includes admission to the Symposium, the Abstract Book, and meals (including gratuities) as outlined in the agenda including the Welcome Reception. You may purchase guest tickets to the Welcome Reception for $50 per person. Registration for the Symposium Gala is optional at an additional cost of $125 per person.
 

The symposium registration fee does not include airfare, ground transportation, housing, gratuities for personal services, or any other costs not listed above.

 

Cancellation Policy


Cancellations received by Friday, 16 July 2011 will be refunded less a $100 administrative fee. No refunds will be given for cancellations received after Friday, 16 July 2011.

If for any reason the Symposium should be cancelled, HCV 2011 will refund the registration fee in full within 90 days of cancellation. The Symposium is not responsible for the refund of airfare, housing, or other personal travel expenses.

 

Substitution Policy


If you are unable to attend the Symposium, you may transfer your registration to another person by notifying HCV 2011, c/o Conference Solutions in writing by email, fax or mail. The following rule of access to the Symposium applies to all delegates, including substitute delegates:
 

Participation in the Symposium is limited to those actively interested in HCV-related research, either at the academic, governmental, private sector or community level. The Organizing Committee of HCV 2011 reserves the right to refuse registration to anyone not meeting this requirement.

 

Liability


With paid registration, delegate acknowledges that he/she has no right to lodge claims against the Symposium organizers should the meeting be altered, hindered, or prevented by unexpected political or economic events, unpredictable forces of nature, speaker cancellations or other unforeseeable events.
 

Letter of Invitation


Delegates who have registered for the Symposium may request a Letter of Invitation to assist with their visa application to attend the Symposium. Please send your email request to Register@ConferenceSolutionsInc.com and include the following information: Name, Date of Birth, Passport Number, and Country of Citizenship. Information on U.S. visa requirements and visa application can be found on the website at http://travel.state.gov/visa.
 

The Letter will then be issued and may be presented to your Embassy/High Commission to assist with the application of your visa. However, this, in no way constitutes an offer to cover the cost of registration, travel or any other expenses.
 

Badges


Symposium identification badges are mandatory and will be required for admission to all symposium functions.
 

Certificate of Attendance


A Certificate of Attendance will be provided to each delegate.

HOW TO REGISTER


Full payment in U.S. dollars (USD) is required to complete registration. Registration is not complete until full payment has been received. 


For payment by check, please complete and submit the online form. Print a copy of your registration confirmation and mail it with your payment to the address indicated in the confirmation message. Check payments must be issued by a bank with branches located in the United States.


For payment by bank wire transfer, please complete and submit the online form. An invoice will be sent to you via email containing the bank account information needed to arrange the funds transfer.  Only non-U.S. institutions may request to pay by direct bank transfer. A $35 processing fee will be added to your invoice.
